Job Description
The YNA Community team is growing and we are always keen to hear from experienced Registered Nurses interested in Clinical Nurse roles, in our team. We have opportunities for experienced, passionate & motivated Registered Nurse leader with demonstrated experience to support the YNA clients with high health and complex needs living in the community. This is ideally a full-time role, based from Adelaide.
The purpose of this position is to:
- To case manage a portfolio of direct care paediatric clients;
- Ensure client centred, individualised care, positive client outcomes & enhance the care of clients in the community;
- Coordinate & support the provision of comprehensive multi-disciplinary care involving assessment, prevention, referral & health promotion;
- Be a valued part of the community and clinical nurse team
- Actively attract & retain health & support staff for the YNA team; and
- Establish & maintain professional relationships with staff, clients & their support network.
About You:
Successful applicants will demonstrate:
- AHPRA registration as a Registered Nurse
- Relevant experience to build the capability to support complex paediatric clients living in the community
- Exceptional communication skills & attention to detail
- A passion for providing quality health care & community support
- Current, unrestricted Drivers Licence
- Current Police Clearance, WWCC & other Clearances required
- Ability to coordinate & lead a team (prior leadership experience preferable).
